Initial Appointment
If you are coming in to our office for the first time, please make sure that you bring the following information:
- A form of photo ID (Drivers license)
- An up to date insurance card

A list of all medication and allergies
- Information about all pertinent personal and family history, including a full cancer history
- All mammogram, ultrasound, MRI films and reports as well as any pathology reports (if you have had a previous biopsy)
