Initial Appointment
If you are coming to the office for the first time, please make sure that you bring the following information:
- An up to date Insurance Card.
- A Form of Photo I.D. (such as a Driver's License).
- A list of your medications and allergies.
- Information about all pertinent personal and family history, including a full cancer history.
- A copy of all radiological tests and reports (when applicable).
If you are coming for a diagnostic breast biopsy, please be sure to discontinue all NSAIDs (such as Motrin, Naprosyn, Aspirin), blood thinners (such as aspirin and Plavix), and Vitamin E for approximately one week prior to your scheduled appointment. Please call the office if you have any questions about your medication of if you believe that the interruption of your medications will cause you medical harm. Consult your Cardiologist or Personal physician if necessary.
For your convenience, a registration for is available to download from this link. It can be filled in at your leisure in the comfort of your home, and brought to the office on your initial visit. We look forward to serving your Breast Health needs.
